Write, sign, and share session records

Updated August 14, 2026

Who can use this

You can add session notes and share documents when you are the supervisor in the supervision relationship.

Session notes belong to a specific meeting. The Documents tab holds files shared across the relationship, such as supervision logs, verification forms, contracts, and board forms.

Add or edit session notes

  1. Open Sessions, select Completed, and open the session.
  2. Select + Add notes when no notes exist, or use the edit control in the notes section.
  3. Complete the note fields shown for that session.
  4. Select Sign here and complete the electronic-signature step.
  5. Select Save notes.

When you add notes while first recording attendance, the notes and signature are submitted together with Submit session. A supervisee can review the completed notes and add their own signature.

Share a relationship document

  1. Open Supervisees and select the supervisee.
  2. Select the Documents tab.
  3. Select Upload a document, or Upload another document when files already exist.
  4. Choose a PDF, JPEG, or PNG file no larger than 10 MB.
  5. Review the selected file and select Upload.

Both you and the supervisee can open files under Your shared documents. You can use Rename or Delete only for a file you uploaded. Files added by the supervisee remain under their control.

Visibility and corrections

Employer admins may be able to view session notes when their role and program settings allow it. Files on the relationship Documents tab are not an employer document center and do not appear there automatically.

Session notes are editable only while the session correction window is open. Shared documents can be renamed or deleted later as long as you are the person who uploaded them.
