Sign required agreements and find signed copies

This page is available to supervisors whose application requires Motivo agreements. You can sign during onboarding or later from your account.

Sign the agreements

The documents open inside your Motivo account, so you do not need a separate Dropbox Sign account.

  1. During onboarding, open Agreements. After onboarding, select Settings > Agreements.
  2. Review the document shown in the embedded signing window.
  3. Complete each required field and apply your signature.
  4. Submit the document. If another document is required, it opens next.
  5. After every required document is signed, select Continue during onboarding.

Newer supervisors generally see one combined document named Motivo Attestation of Accreditation + Business Associate Agreement. Supervisors who began the process under the earlier document set may instead see Supervisor attestation and Business Associate Agreement.

If you leave before finishing

The signing window does not offer a cancel action. If you close the browser or navigate away, return to Agreements. The page resumes with the first unsigned document.

The onboarding Continue button stays disabled until all required documents are recorded as signed. If you signed a document but the next step does not become available, reload Agreements and confirm that the completion message appears.

Find your signed copies

After signing is complete, the page confirms that the documents have been emailed to you. Keep that email and its attachments for your records.

The current Agreements page does not provide a download button for previously signed files. If you cannot find the email containing your signed copies, contact support@motivohealth.com and include the email address on your Motivo account.
