Add or update licenses and credentials

You must be signed in as a supervisor. Independent supervisors see Licenses & certifications; in-house supervisors see Credentials with a smaller set of fields.
Add a license or certification
Use one entry for each state and credential type you hold.
- Select Settings.
- Open Licenses & certifications. If you are an in-house supervisor, open Credentials.
- Select Add a license or certification or Add another license or certification.
- Choose License or Certification.
- Enter the state, type, number, issued year, expiration date, and requested document.
- Answer Are you able to supervise for this license? If you choose Yes, describe how you meet the state's supervision requirements and add requested training documents.
- Select Save changes at the bottom of the page.
Certifications require a file. A license file may be optional in the form, but other supporting documents can still be required for profile review or a specific state.
Add other credentials
Independent supervisors can use Supervision credentials to add a state, credential type, credential number, certificate, and expiration date. Use Telehealth training documents and Liability insurance for those separate records.
In-house supervisors use Education for degrees and Other credentials for supervision credentials. Their employer can view the credential information connected to the account.
Correct an existing entry
Open the entry and use its edit control for fields that remain available. Some core fields on an existing license, such as state, type, or number, may be locked. When the form directs you to delete and reupload the entry, first gather the correct information and files, select Delete, then create a replacement entry.
Deleting a credential can affect whether your profile is considered complete. Add the replacement and select Save changes in the same visit when possible.
Saving updated information does not verify it immediately. Motivo reviews licenses and credentials manually, and a material change may require another review.
